Liability Waiver, Release & Assumption of Risk
I acknowledge and understand that participation in soccer training, camps, and related activities involves inherent and significant risks, including but not limited to falls, collisions, contact with other participants, equipment-related injuries, and other unforeseen risks that may result in serious injury, permanent disability, or death.
By registering my child, I voluntarily and knowingly assume all risks associated with participation. I hereby release, waive, discharge, and agree to hold harmless the camp, its directors, coaches, staff, volunteers, facility providers, and affiliates from any and all claims, liabilities, demands, actions, or causes of action arising out of or related to any injury, illness, loss, or damage that may occur as a result of participation, whether caused by negligence or otherwise, to the fullest extent permitted by law.
I confirm that my child is physically fit to participate and that I accept full financial responsibility for any medical treatment that may be required.
